JACC is a State-funded program that provides a broad array of in-home and community-based services to individuals age 60 and older who meet clinical eligibility for nursing home level of care and who desire to remain in their homes within the community. Individuals who meet clinical eligibility for nursing home level of care are individuals who require assistance with a minimum of three activities of daily living ADL such as bathing, toileting, dressing, transfers and locomotion.
The JACC program is designed with the goal of avoiding diverting or delaying the placement of the individual in a nursing facility. JACC aims to strengthen participants' networks of informal caregivers and to maximize autonomy by providing participants with the opportunity to direct their own care, including hiring their own providers, if desired. What services does JACC offer? All JACC participants receive care management services. A plan of care is created for each participant as the result of a collaboration between the participant and his or her care manager.
With the participants needs, desires, and goals in mind, the plan of care specifies additional services to be delivered, which may include:. Are there limits to JACC services? A monthly cost cap is applied to JACC services for each participant.
The service package provided is based on an assessment of the participant's needs, unique plan of care, and availability of services and funding. Who provides these services?
