We believe in building a su pportive community linking with other care organisations, the local council, other health care professionals and ne tworks to ensure every individual has a truly person centred experience with our service and has the support to achieve the best outcomes.
Many individuals wish to stay in their own homes, and enjoy care at home, wh ere they fe el co mfortable in their surroundings. Individuals should be able to choose what home care th ey would like and how they would like it delivered.
We ensure the individual is at the forefront of the decision making about their care, and have access to factual information to base these decisions on. The package of care is person centred to every individual, ensuring each one fulfils the individual needs and wishes, while giving them the support they require. We encourage and promote independence and work with the individual, but do not take over.
We work closely with other healthcare professional and can organise referrals for individuals if they require and consent. We are open and transparent with our home care and companionship clients and always value their feedback. We regularly visit our fabulous clients that live in nursing homes. We assist clients to go to appointments, shopping, trips out and much, much more. As befrienders, we chat about their favourite artists, memories and encourage fun and laughter!
