LIFE Pittsburgh arranges a full array of health and social services 24 hours a day, seven days a week, days a year. The team then plans and approves services with them and their family or caregiver, monitors for changes and provides timely interventions. All services provided or arranged by LIFE Pittsburgh are fully covered when approved by the health team.
Our Participants will receive a service package specifically designed to meet their needs. Some examples of these services include the following, which are based on individual needs:. All Participants meet with the physician, nurse, social worker, dietitian and Certified Therapeutic Recreational Specialist prior to enrollment and at least every three months to develop a plan of care designed specifically for them. Care plans are tailored to the individual and will be reviewed as frequently as necessary, or at the request of Participants and family members and adjusted by need.
